Proof Decision Weirdness, any clue?

Yesterday a new version of a proof went out and everybody had the default decision pre-populated for "Not relevant." So first, I've never seen any decision pre-populate before. And 2nd, "not relevant" is hidden as a decision option so shouldn't even be a selection at all. It doesn't show up as an option when you make a decision on that proof. Everyone who approved showed up as "Decision Changed to Approved" notification. The designer who sent out the proof has no idea what he did differently. Anybody know what happened so we can avoid it again in the future?
