I am seeing in both Sandbox and Production a glitch/bug where occaisionally an old style version of a custom form is displayed to the user. This is in turn causing some of our Fusion scenarios to not complete as expected as the fields and answers to them that decide various pathways in Fusion are not being filled.
Is anyone else experiencing this?

can you say more about this? I'm struggling to understand whether you're talking about how the current form looks, or whether you are saying that it's an older version of the form with some questions missing?

what I have seen is theat the User fills in a custom form as per usual, everything looks ok, all working as normal etc. But when they hit Save (usually a blue button in the bottom left corner) rather than it just saving an moving on, instead there is a pop-up with the same custom form (same questions etc) then show but in an old style format with orange colouring. See the attached screenshot for what pops up. You then have to click the same selections in this version of the form and hit save (a rectangular orange button this time) in order to move on with the process.
When this happens, it messes up our Fusion integrations as the answers to these custom fields are not being captured.
This is happening intermittantly, I'd say about 1 in every 4 cases (though I haven't extensively tested this so I can't give an accurate number on how frequesntly this is occuring.)
Other colleagues have noticed this too, so I don't think it is something weird being done by the user but rather a bug on Adobe's side.

ok thanks for the clarification, I just was having a hard time picturing it but this makes it more clear. And then, just because I see your screenshot is on a dashboard -- what exactly is the user doing on that dashboard to get a custom form to pop up? Are they editing one of the report rows? (what is it a report for?)
Either way, it seems exceedingly weird and since you're able to replicate it 25% of the time, I hope you find a moment to record your screen and send in the recording for a Support case.

I have a upport case open already and have asked a member of the Adobe support staff to contact me to book a meeting time so I can show them when/how this error is being triggered (my words to them were - this is quite a complex workflow we have in place and I think it is difficult to replicate without being shown exactly what our users do so I doubt you can re-create the error, please book a time with me and I can show you the process)
