Does anyone know if it's possible to customize this form so that I can have the description field displayed? Ideally I would like Name, Description, Assign to. I know that I can go to 'Requests -> Requests I've made' to edit the requests which I've submitted to add this info but it seems like a bunch of unnecessary clicks.
