Has anyone used Workfront to collect data for multiple documents submitted through a Request?

If I set a limit of 5 documents per submission, the custom form would have to have duplicated fields, ie. Doc1 - Audience, Doc2 - Audience, Doc3 - Audience, etc. Any way around that? Any suggestions? Thanks!
