How can I add an URL (clickable ) within a custom form. For example: If you answer no, you can fill out the form by accessing this link.

Adding the "descriptive text" field to your custom form will provide you with a field that you can put your URL in and it automatically makes it clickable for you. It's a little clunky but pretty intuitive.
