Reporting: Add Template Name back to Template Task ID Selection

Previously, the template name was included with the Template Task ID selection for reporting. This allowed us to select the appropriate template task when we had multiple template tasks with the same name. We are aware of the workaround of using the Task Name and Template ID filters together; however, this is not ideal as our PMs sometimes change the task name after a project is created.
