Creating frequency data for checkbox options

Hello, 

I have a custom form with checkbox options for colleagues to select the items needed for a project. I want to create a report counting the frequency of each item. I couldn't create a matrix grouping report, nor can I create a chart with checkbox field. How should I create the custom form so that I can do reports on the item frequency?
