Yes we have installed aem-service-pkg-6.4.1.zip + AEM-FORMS-6.4-WIN-5.0.86.zip in all the instances. Also I have tried with the AEM-6.4.3.0-6.4.3.zip + AEM-Forms-6.4.3.0-WIN-5.1.42.zip in my local instance and had the same issue. Are you referring any different forms package ?
