As I am using AEM forms in CS, I added this script under /apps. in clientlibs folder in js file and added categories property. but this script is not loading in forms getting error as belowhere what needs to be done?

How to populate values in dynamic dropdown in AEM forms?for eg- Country/ State/ Cities in three dropdown dynamically.Which is best way to achieve this functionality
