did you verify the file permissions? Also, please make sure the user which is used to run Apache has proper access on the file. can you please share the error you're getting when you use IncludeOptional?

All the OOTB workflows are stored under "/libs/settings/workflow/models" and If you make any changes in the existing workflow model, then changes will be stored at "/conf/global/settings/workflow/models".   Hope this helps.
