Check the clientlib category in Adaptive form container dialog (it should be same as your clientlib category).And after that also you are not seeing then change the function like this.function funcName(){your logic.}

This issue is causing because of some wrong code / unsupported code in your clientlibs. Check your clientlibs js (if many files remove one by one on js.txt to find unsupported js file. If only one file then remove each method to find the error / unsupported code in js). 

Resolved the problem by adding the category in AEM form container component's formContainer.html 

For my case, i have create the custom client library and add that client library category in theme field.After deployment that value is empty in theme console (becoz of that css is not present in form page), but present in crx.Thanks in advance

Hi Vijay,Thank you for your reply, i found the solution. (checkbox in dialog).

Found the solution, there is a checkbox field in dialog - "Form covers entire width of the page"this needs to be unchecked whenever we need to use the multiple forms in same page.If the above checkbox is unchecked, we can use the multiple adaptive form (using the Adaptive form embed) component.
