Hi @Richard__Carlson , thats a great question, I should have explained the rationale for not including the form in WF in my original post.  There is logic built into the form that will not allow a proof to be generated unless baseline questions are answered by the user creating the proof. 

We have attached a custom form that users have to fill out when generating proofs in Workfront.  The data in that custom form is stored in ProofHQ.  It would be useful if the data from those forms could be referenced and reported on from Workfront.  Currently, that data is inaccessible for reporting...
