You are trying to do this using AEM Forms JEE and not an AEM Workflow correct? If yes then I've attached an LCA that you can import through adminui that is working on my AEM Forms 6.2 JEE install. Rename the file from .txt to .lca to do the import
I'm confused, you say you're using AEM Forms meaning you've installed the AEM Forms Addon package and are developing elements such as Adaptive Forms. Are you certain of this or are you using the default forms capabilities that comes with AEM?

If the header and footer are the same why not create a template with the header and footer or create form fragment header/footer elements. Using fragments allows you to modify them as needed and have the changes replicated automatically across forms.
I'd be interested in hearing the use case where the rule builder wouldn't suffice to hide/show a panel. Also is this a common element that a fragment wouldn't be a potential option?
What OS are you using? Is this a JBoss turnkey install, if so the installer will install everything it needs. If not what appserver and DB are you using?
