I created a form with html and php and I would like for the data once submitted to be placed in a PDF form that I created. I used a tutorial that I found online

The reason I know that it is working is b/c I used that PDF file that the website used, but if I use the form I created or modify the file online it doesn't work. Can anyone please help. Is there something I must do to the form I create for data to be imported. I am using the new CS3, could this be a problem.
