Online HTML or PHP form to emailing PDF form

Hi,

I want the submit button in a online html or php (whichever works) form to make a pdf form and fill out the fields accordingly and then send the pdf form by email.



Is it possible? I am a newbie to Adobe forums and if this stuff is discussed before can you link me to the older topics please.
