I am getting a symbol of a white photo with a tear across it and a bent corner that appears in the place of a presumed edited photo in my organizer after I have edited it either saving as a version set or not. What is going on and what can I do to fix it

I am getting a symbol against a grey background of a white photo with a rip across it and a bent corner that appears in the organizer after I have edited the photo whether I have saved it as a version set or not.  What is going on and how can I fix this and get my edited photo back? This hasn't happened to me over 4 years of use.
