The reason for the discrepancy is probably because of the way your product collects data on form completions rather than visits. If the sessions are recorded differently or if multiple forms are completed in a single session, you may observe less completions while viewing by visits.
On the other hand, the CJA comparison of "people" (unique visitors) vs "sessions" indicates a consistent sum for form completions. Make sure that sessions and completions are consistently tracked in your fallout visuals. This is important because different dimensions can lead to variations in reported metrics.
I would suggest double-checking how your analytics tool handles these dimensions and aggregations.
