Server Cluster Map with Performance Metrics

I am currently working with some very old hardware (2005) that is choking.   New hardware coming soon (data validation phase).

Keeping this old cluster going is creating some challenges.  What would be nice, is to have a Server Cluster Map with performance /communication indicators to that we can easily id the bad server.   Ideally, would also like to see some suggested steps to deal with the most common issues.

I know we have a 'server monitor' but a map would be better.
