Reporting on Defects

I have a need to identify the number of rounds a task is worked within a specific project - specifically related to approval loops. That is, I'd like to track the number of times an approval is rejected before it is approved. Is there a report that can aggregate this data or, at minimum, a way to see the data manually within each individual project? Christian Blasberg Lincoln National Corporation

I'm not sure if this will help but I worked with Support to figure out a report that would show me the number of revisions for any given document. For us, revisions roughly equates to review and approval steps.
