Filtering on Document Version Report - for Task Status and Task Assignee

I have built a Doc version report, but I would like to add two filters so the user only sees proofs where the task is assigned to them and has been marked with a "Changes Needed" status. Can anyone help with the text mode instructions I need to use?

 

Goals:

 

Filter1:

Task:Status = Changes Needed

 

Filter2:

Task:Assignee = $$User
