Move upload field

Add ability to move upload field to end of forms/issues.

Being able to move the upload field to the end would be a good start; but ideally would want to be able to place it anywhere in the request (ditto for subject field, priority, etc.)

There are situations we would keep the upload field near the beginning, but want to place a descriptive text section above it to convey request instructions.
