Improved text readability in request forms

A number of our users have complained that request form text is too small and lacking in contrast. Perhaps bumping up the pt size and contrast would help?

Example comment...

"As long as I am making suggestions, here is a comment about this web page. The default size of the font is too small and the grey color of the font does not have enough contrast to make it easy to read."
