Hello - the request form for the free upgrade that's linked at the help page you gave above: Upgrade Adobe Designer | Acrobat XI‌ (form is listed under "how do I claim my upgrade") does not work. It just sends me to the support page, which then loops back to the help page, and so on and so forth ad infinitum. Can you provide an alternate link for the request form?
