Turning PDF into a Web Page

I have an extensive PDF form created with LC Designer, and I would like the form to be fillable on the internet.

What is the best way to go about doing this (I do have web hosting, but don't know much when it comes to HTML or XML or XDP...)?

I'd prefer cheap/free solutions whenever possible.
