how to change logo for any form in live cycle designer

iam trying to change the logo of a particualr or any other form in live cycle designer ES2.Could anyone tell me how to chnage the header and footer of any form ?

Those operations are done on the Master Page(s) in Designer.

yes but is there any one who helps me to add header and footer to my forms.How this can be done in master pages.
