Expandable Table Help Needed

Have created a sample table per instructions here, http://forms.stefcameron.com/2009/02/25/expandable-table-with-totals/, and modified to suit the need, but the 'add/remove' buttons are not working (nothing happens when I click them except the screen flutters).  They work great in the sample, but I don't know what I've done wrong to cause the problem.  Any way to post the sample I'm working on?  Thank you!

You can use Acrobat.com website to upload the document and post the link in this forum.
