Create vcard qr code

I would like to create a QR code in vCard format from different fields. Now I have the problem that my QR code only works in the design view. Once I open the PDF in Preview or in Reader, the QR code looks different, and scanning via app does not work any more. Can someone help me?

here's a custom QR-Code solution that works in Reader.
