Adding Calculations to PDF forms

I have reviewed the info in Help regarding FormCalc and can't seem to make any progress. I have three columns: Number of Times (selected or entered by the user a value of 0 through 4), Mean Value (which is set value that is multiplied by the Number of Times, and Total (each row will have a total Number of Times multiplies by Mean Value).



I have been able to create a drop down list with the numbers 0, 1, 2, 3, and 4. But there is test with the title of the object type I can't seem to get rid of.



Once these lines are all answered, I need to return a grand total of all of the totals in the right column.



Works great in my spread sheet but when I convert to pdf I lose all of the entry fields (really only the Number of Times column)



We have the blind leading the blind here and I have been scouring the web and have come up with nothing I can understand or am I able to apply to my problem. Help!!!

This should read - "But there is TEXT..."



The title of the object type is the object type as called by Adobe LiveCycle.
