Workflow Customization

I want to add a date and time picker to a start workflow dialog. Fo eg. An author comes to trigger a workflow. And as soon as he select an asset and select WF the Start Workflow dialog appears. There are 3 field to that page

1)Workflow

2)Comment

3)Workflow title

I want to add an extra date time field in this dialog.
