From where Add Folder popup is coming while creating Folders in Assets console of AEM 6.2

Hi,

I got a requirement like I need to hide the name filed in create folder popup in AEM 6.2. I tried to find it from where this pop up is coming from. But, I could not get it. Can any one help me from where this popup is coming from?
