I would like to have users be able to fill out a form on my website and then have the data sent to a database server.  The form needs to look like the pdf form it's based on. I was wondering if this was possible and if there are any examples of this?

Can you help me explain what do you mean by "form needs to look like the pdf form it's based on"?

Please confirm if you have AEM forms server or AEM forms designer, based on the current software you have we will suggest the next steps.

Thanks,
