create message using form fields value

Hi All.

I new in AEM Form Designer. I would like to create message based on fields value. That means, for instance, message header will Last Name and First Name values related to Last Name and First Name fields in the PDF forms. If that is possible how to do that?

I am not sure what you mean - do you mean you want to read the fields of the form and then  dynamically create a message.
