Boxes with question marks in deliveries

When we build deliveries and proof them we are seeing boxes with question marks in our templates. This doesnt show when i preview in adobe only when i proof to outlook and gmail accounts. Has anyone experienced this and if so what have you done about it? It seems there is a problem with the code.

Is it only happening with 2 mailboxes ? Once you open the email in gmail or outlook could you ensure all images are downloaded

Few questions:

What happens when you display the mirror page?

Did you try copy the inage url and then display it in the browser?

Are all images stored in Campaign?
