It would help if you shared what you are trying to do, what script you are trying to run... 

 

I'm not sure why you need "eval"..  Adobe Custom Code block run JS directly...

Here my scenario is if user clicks others option it will ask one more fields, I need to track that 2 fields values, for that i need proper JavaScript code

I don't see anything wrong with the code you posted above... where did you add this code?

 

Maybe the problem is the location?

 

Can you explain where this code has been added?
