Hi Bruce,
I'm not sure I'm understanding your question fully, do you want to track when the status of your project changes, say when it goes from New to Current or you might have unique statuses like Ready to Assign? So when a user changes the status to something new you want to know the date/time that occurred?

Hey Bruce,
As long as I'm understanding your question correctly - if it absolutely has to be on a project report, you can include the text that was included with the latest note/update that was made. Depending on what suits your needs, you can either use the field "Last Note: Note Text" (which displays the most recent text update entered by any user on the project), or you could try the "Last Condition Note: Note Text" (which displays the most recent update made specifically when the condition of the project was changed).
However, if you want to see a whole history of notes/updates for each project (not just the last one), you may want to try creating a note report instead of a project report. You can still set similar parameters for your filters as you would for a project report, but a note report will allow you to see the entire history of them for each project. Be sure to group your notes by project name so they all stay together, and then you can even identify whether the note is connected to the overall project or if it's tied to a specific task within the project (see the attached screen shot of a test report below).
I hope that helps, but I'm with @Kurt_Jones a bit here as far as probably needing a little bit more information since Workfront can provide a variety of nuanced solutions here depending on what exactly you're needing.
