Text Mode help: Pull issue field to a project report

I would like to customize a report for my requestors that displays an editable field from the custom form for them to edit so it appears on my reporting. To achieve this, they would only be able to edit the field that appears on the original issue.

This means I need to pull that issue field from the custom form on to my project based report.

Can someone help me with a code to achieve that? the field on the issue custom form that needs to appear is "custom report note"
