Request info did not attach to project when converted using a specific template

I converted a request to a project using one of our specific templates. However, the request info did not populate in the project details. Any idea why this may have happened?

Hi @jklemm , I'm not sure, but this might be a Custom Form issue.  I suggest checking if you have a Custom Form that is attached to the request but not attached to the template.
