Proof Approver name and decision date on Document Report

Hi! 

I've got a Document report that is in a desperate need of a column for Proof Approver name and proof decision date. I would try other report types, however this same report also needs to display a bunch of Task and Project level metadata (+ a direct download link for the doc). Any tips/guides on how to achieve it would be appreciated.

You could take a look at this post. Even though it is a document version report (rather than a document report) it seems like you can get task and project information (I see a project name column, as an example).
