Error message: duplicate key value violates unique constraint

Hello - in building a custom form, we did a radio button field, and we get the following error: duplicate key value violates unique constraint

What does this mean?

We get this error now on every new field we are adding to the form.

    Hi Olivia - Are you using the Field Library when adding new fields to your form? You usually get that error message when you are trying to add a field that already exists to a new form. Remember, while you can have the same Label for fields, you do need a unique name for the fields.
