Is there a way to run a report that shows all the places a specific custom form is attached?

You will have to run separate reports for the following objects:

  • Project
  • Task
  • Request
  • Documents

The field you are looking for is "Category: Name"

 

If you add it as a column, you can easily click on the form and see what information is filled out on that specific object. 

I recommend adding it also as a prompt so you can easily filter through the different forms you are trying to potentially audit.

I would actually stay away from the "Category: Name" field and use the "Categories: Name" field.  
The issue with using "Category: Name" in the filter is that it will only include objects where the custom form was attached FIRST.  If the custom form you're interested in is attached after the first form is attached, then the object will not be included in the report result.
