Auto-add custom form to an object when created (Request/Issue, Project, etc.)

We have custom forms that we attach to our Requests and Projects when they're created. Currently this is a manual procedure that each PM must remember to complete. Is it possible for us to build our Request Queue and Project Templates in such a way that the preferred custom forms are attached automatically upon creation?
