Requester Custom Form Additons

It would be super to have the ability for the user to be able to add additional fields to a custom form. For example, I'm asking a user to provide equipment information, and have set out the 2-3 fields I need for each piece of equipment. It would be great if the user could push a +Add new xxx link or button, to replicate the previous 2-3 fields and fill that out, so they could add the 20 or so pieces of equipment directly into the form without the form creator having to create these many blank fields that may or may not ever be utilized.
