Report on Project Approval (and task and issues) dates like Document Approval

I'd like to be be able to report on WHEN a project is approved as well as the USER who approved it and any comments they may have. Not just the role or the approval process that was used. The actual approval dates. Documents have this because there's an additional object added for it but not projects or tasks.
