Include Images in Export PDF of Custom Forms

Description - Include any Images placed inside a custom form when exporting form as a PDF from a Project/Task/Request.  

We quote design work for Retail customers.  I would like to be able to insert a barcode image within a Custom Form created to be used as a Printable Quote sheet for the customer.  This barcode can then be scanned at the time the customer pays.  

Current Behaviour - When exporting PDFs of Custom Forms the images are not included and the field is shown as blank.
