Further customization for default fields in Queue Setup

I want to be able to have the custom form before attach document and other default fields. I also want to make some of the default fields required for request submission.

This sounds like something that can be included in the new Activity Log functionality. Would it be convenient to put it as a separate post in here so we can track it accordingly?
