Allow for custom fields to be repeated on a form

Description - We often need to ask the same question more than once on a form, i.e. 'what size flyer is needed?' or something similar for our multi asset requests.

Why is this feature important to you - This would help automate the multi asset request process.

How would you like the feature to work - allow admins to add the same custom fields to a form without one answer overriding the previous one.

Current Behavior - the same exact fields will show the same answer when filled in across all of them.

Unfortunately, this will not happen. That's just not how databases work.

The only way to do this is to have different fields. You can, however, add a number to the field name if you need more than one: "Flyer size 1," "Flyer Size 2," etc.

This can be addressed today by changing the Name/Label value on a field.

With Flyer 1 information:

Dropdown field with values (US Letter, US Letter Half Page, etc) - Label = Flyer Size, Name = Flyer Size 1

With Flyer 2 information:

Dropdown field with values (US Letter, US Letter Half Page, etc) - Label = Flyer Size, Name = Flyer Size 2

This can add a lot of maintenance to the system when you need to update fields but is a way to capture this data while providing a streamlined experience to the requester.
