Want to create Issues everytime a new document is uploaded in a campaign

How can I use Fusion to create issues every time a new document is uploaded in a specific campaign? 

I do not want the same document to go through the scenario more than once, only new documents uploaded to same campaign. Any suggestions on the best way to do this would be greatly appreciated.
