Covert Issue to Project - Hiccups

Hi there,

I'm struggling with two things - my custom form data from the issue carrying over into the newly created project, and, mapping assignments to that new project.

The template online doesn't get into that detail. 

Does anyone have a blueprint to share? Or screenshot of the scenario?
