Custom Form - Capture multiple address with a Company Object

Hi,

I want to create a custom from mapped with Comapny object. This form should have provision to capture multiple address of a company.

Example:

Company: XYZ (WF standard field)                                    (Parent)

-------------------------------------------------

Address (Custom Form)

A-23, Block C1, Street D4, Town, State, Country-US         (Child-1)

B-23, Block C2, Street D5, Town, State, Country-UK         (Child-2)

C-23, Block C3, Street D6, Town, State, Country-FR         (Child-3)
