critical help needed for prod, our aem forms header defined in our form template goes to bottom, which the same template and theme, works on on dev

Hi, we have a AEM adaptive form in 6.5.8, everything works fine on dev. But when we package everything and deployed to PROD, the header defined in form tempate shows up at the bottom of the form. We compared the content.xml line by line for all the forms/fragments/folders, other than replication status and jcr:uuid, everything is the same. Any idea? See images attached.

On our dev, you can see the header is showing correctly:

On PROD, you can see the header defined in the form editable template went to bottom.

Also, the guideimage in XML works fine in DEV, but not in PROD.

You need to make sure all related artifacts are pushed to the production set-up including the custom fonts (if used) or any custom components used in dev. 

Error logs may also indicate the missing artifacts.
